Are you "pro se" ( or Pro per) , without a lawyer? There are no "motion forms". There is a format.
If you could tell me what type of case you are involved in? And what is it you want the court to reconsider, I could guide you as to format and content.

A "motion to reconciser" a ruling, is not a "form" nor at anyone's fingertips. To recondider a ruling requires an undertsanding of under what theory or law the judge CAN reconsider and why s/he should or could do do.
It was may understanding that the intial filings are already complete ( a done deal) and the case has progressed to a court ruling, which is what the "asker" is trying to "appeal".
Of course, if s/he is "pro se" , s/he could always just simply put the request in plain english without any legal support for the motion. Those type of motions do not usually succed. I do not want this case to enter the realm of res adjudicata ( if it hasn't already) by using up his / her chance by a faulty filing.

Call your COURT HOUSE in your state; ask for Family Services ; they will more than likely have a COURT SERVICES CENTER ; you can actually go to in person and speak to someone that works in that CENTER to service individuals such as yourself filing on their own....I am in CT and am in the process on my own..and forms and info are right there at my fingertips.
